Several dermal substitutes are available on the market, but there is no precise indication that helps surgeons choose the proper one. Few studies have tried to compare different xenogeneic bioengineered products, but no objective bio-parametric comparison has been made yet. Fifteen patients who underwent skin reconstruction with Integra® or Pelnac® were retrospectively evaluated. After at least 12 months of follow-up, an objective and quantitative assessment of several skin biophysical properties, such as color, texture, elasticity, hydration, glossiness and trans-epidermal water loss, were measured with non-invasive skin measurement devices. The grafted skin showed a reduction of the superficial hydration level and a tendency to lower values of trans-epidermal water loss with both dermal substitutes. Melanic and hemoglobin pigmentation were higher in comparison to the donor site in both groups, while a melanic pigmentation increase versus the surrounding skin was seen just with Integra®. Finally, the skin was found to be more elastic when reconstructed with Integra®. The skin barrier appeared to be intact in both groups. Hence, these substitutes are valuable means of skin regeneration. Integra® seems to be more advantageous for reconstructing areas that need more skin flexibility.

BACKGROUND: Acne is a frequent adolescent disease characterized by inflammatory and non-inflammatory lesions whose topical treatment very often presents adverse phenomena such as irritation or resistance to antibiotics that reduce the patient's compliance. The purpose of this study is to compare a commercial product (Acnatac gel) based on clindamycin-tretinoin (CTG) with a galenic compound containing 2 essential oils (Myrtus communisL. and Origanum vulgare) and tretinoin (MOTC) to evaluate its anti-acne effectiveness and action on the microclimate of the skin. METHODS: Sixty volunteers were randomly divided into an A group using MOTC and a B group, as a positive control, using CTG. The effectiveness was assessed with non-invasive skin analysis (Sebumeter, pH meter, Tewameter and Mexameter) and the counts of the number of lesions, after 15 and 30 days. RESULTS: In both groups, there is a worsening of transepidermal water loss (TEWL) due to tretinoin. MOTC has improved, starting from 15 days of treatment, the papular erythema (p = 0.0329 vs CTG) and has reduced at all times even the rashes of retinoids present in the healthy perilesional skin (p = 0.0329 and p = 0.0017, respectively, at 15 and 30 days). CONCLUSION: MOTC has shown, compared to Acnatac, to have anti-acne efficacy and to possess an anti-inflammatory activity, due to essential oils, able to reduce in vivo erythematous lesions and those induced by retinoids.

INTRODUCTION: Psoriasis is a chronic inflammatory disease characterized by the presence of erythematosquamous lesions. A wide variety of topical treatments for therapy of this pathology are available, including sodium bicarbonate (SB). A few papers reported in literature focus on use of SB baths for treatment of psoriasis, but none assess evidence concerning the efficacy of SB topical preparations. This study aimed to determine the effectiveness of a galenic SB in lanette vax formulation compared with lanette vax base in mild to moderate stable plaque psoriasis. METHODS: A randomized, double-blind, intrapatient, controlled study was performed in 28 days. Thirty patients of both genders were selected for testing. A blinded investigator evaluated the patients' psoriasis using a modified Psoriasis Area and Severity Index (PASI), body surface area (BSA), and objective parameters using sensors (Multiprobe Adapter MPA5; Courage & Khazaka Electronic GmbH, Cologne, Germany). RESULTS: Data analysis of objective parameters highlighted that use of the SB topical preparation led to no improvement in skin hydration, no reduction in transepidermal water loss, and no decrease of erythema. The modified PASI and BSA did not change from baseline. CONCLUSIONS: The results obtained show that use of the studied product did not improve psoriatic lesions.

BACKGROUND: Research results on the efficacy of sunblock sticks in avoiding or reducing Herpes labialis (HL) recurrences are contradictory and has shown mixed results, with some protection reported under experimental conditions that could not be replicated under natural conditions. OBJECTIVE: The purpose of the present work was to carry out an in vivo test on the effectiveness of a protective lip product in preventing recurrent HL in natural conditions during summer. METHODS: An exploratory randomized crossover study was performed on 20 adult volunteers who served as their own controls during the analysis. The study was conducted between May and July 2017 around the summer solstice. During the two-month period, volunteers had to perform normal daily activities and also needed to go to the beach in Sardinia; thirty subjects applying SPF 30 protection and 30 without protection. RESULTS: In individuals with stick protection, the number of HL attacks was significantly reduced. Based on association between different parameters, which emerged from the medical history and the post-test questionnaire, it was found that male individuals over 44 y of age and with Fitzpatrick skin type 2 were the most exposed to recurrence. CONCLUSION: HL recurrence rate can be reduced by using sunblock stick in summer.

BACKGROUND: Adhesive pads should reduce the action of the local muscle contraction on the skin leading to a decrease in the depth of existing wrinkles and the formation of new dynamic wrinkles. AIM OF THE WORK: This study aims at assessing the antiwrinkles action of adhesive pads during time, and the temporary improvement of facial skin appearance by reducing the vision of linear wrinkles and improving skin elasticity. PATIENTS AND METHODS: Thirty-nine subjects participated to a placebo-controlled study. In the short-term test, the measurements were taken 15, 30, and 60 minutes following 30 minutes application of the product; in the long-term test, the measurements were taken after wearing pads every night for 4 weeks. The roughness parameter of the skin surface was calculated by using a profilometry software 3D MEX® . RESULTS: In the short- and long-term tests, analyzing the average of the elastomeric measurements, no significant change was observed in any of the parameters analyzed after 15, 30, and 60 minutes. The adhesive pad decreased significantly all roughness skin parameters 15 minutes after short-term application and until 60 minutes after long-term application. These changes did not occur in the contralateral untreated zone. CONCLUSIONS: The use of topical adhesive pads improves wrinkles in the crow's feet area in the first hour after use. However, patient self-evaluation indicated that the use of topical adhesive pads for 3 weeks may offer subjective improvement in crow's feet zone over a 2 hour period. Topical adhesive pads are safe to use and tolerable for most users.
